Horizon Planning

Horizon Planning® is WellSpan’s name for advanced care planning, or making health care decisions ahead of time in case something happens and you’re no longer able to. We encourage every adult to make a Horizon Plan, regardless of age or current health. This section includes information on starting the conversation with your loved ones and also documenting your wishes for care.
